Assist-Life Stage, Transition

Navigating major life changes can be overwhelming. We provide the guidance and support you need during key transitions in life to ensure they’re smooth, empowering, and well-planned.

We help with transitions such as:

  • Moving out of home into supported or independent living

  • Transitioning from school to adult services or employment

  • Starting a new job or community activity

  • Adjusting to a new routine or support structure

  • Future planning for long-term goals

Our support includes mentoring, decision-making assistance, coordination, and emotional support tailored to each life stage.

Assist-Personal Activities

We provide respectful and reliable support for personal daily activities, always ensuring that your dignity, privacy, and preferences are honoured.

This service includes assistance with:

  • Showering and bathing

  • Dressing and personal grooming

  • Toileting and continence support

  • Meal preparation and feeding

  • Taking medication on time

  • Mobility and transfers (bed, wheelchair, etc.)

Our aim is to help you maintain your personal wellbeing while building independence wherever possible.

Assist-Travel/ Transport

We understand the importance of being able to get to appointments, activities, or social events safely and independently. That’s why we offer travel and transport support tailored to your needs.

Support may include:

  • Transport to medical appointments, therapy, or school

  • Travel to work, job interviews, or training programs

  • Assistance getting to community or social activities

  • Learning how to use public transport confidently and safely

  • Help with organising accessible transport options

Whether it’s short trips or regular travel, we make getting around easier and more reliable.

Daily Tasks/Shared Living

We assist individuals living in shared or supported housing to manage their daily routines and participate fully in household life.

Support covers:

  • Planning and preparing meals

  • Support in communication and cooperation with housemates

Innov Community Participation

Development-Life Skills

  • Personal hygiene and daily routines

  • Social and communication skills

Household Tasks

We help participants maintain a clean, safe, and functional home environment. This service is especially valuable for those with limited mobility or health conditions that make housework difficult.

We provide assistance with:

  • General cleaning (vacuuming, mopping, dusting)

  • Dishwashing and tidying up

  • Laundry, ironing, and folding clothes

  • Bed-making and linen changes

  • Shopping for groceries and household items

  • Light garden maintenance (optional)

We ensure that your living space supports your health and comfort.

Participate Community

We encourage participants to get out into the community, meet new people, and enjoy social activities that promote inclusion, confidence, and wellbeing.

Support includes:
